- $35 CoPay for Primary Care doctor’s office visits (unlimited)
- 100% Coverage for Preventive Care
- $50 CoPay for Specialist doctor’s office visits (unlimited)
- Yearly Vision Exam (1 every 12 months)
- $250 CoPay for Emergency Room visit
- $50 CoPay for Urgent Care visit
- Co-Insurance: 80% for deductibles $750 to $5,000 and 100% for deductibles
$7,500 to $20,000
- Out of Pocket Maximum In Network: Deductible + $2,500 for deductibles
$750 to $5,000, and Deductible + $0 for deductibles $7,500 to $20,000
- Unlimited lifetime maximum benefit
- Mail Order Rx available
- Maternity available with deductibles of $2,500 or greater for additional
cost (12 month waiting period applies)
- PPO network available for this plan with slightly different benefits at a

- Tier 1: $15 CoPay; Tier 2: $30 CoPay; Tier 3: $60 CoPay; Tier 4: 25% Co-Insurance with a $2500 out-of-pocket maximum per person per year.
