1500 Plan:
$40 Copay (unlimited # of visits)
3000 Plan: $30 Copay (4 visits per year)
5000 Plan: $20 Copay (4 visits per year)
- $100 CoPay for ER Visits
- Blue Cross PPO Network of Doctors
- 100% Co-Insurance
- Out of Pocket Maximum: Deductible + $0
- $5 million lifetime maximum benefit
- 9 month Initial rate guarantee
- No Maternity coverage
- Up to $500 annually is paid for Dental
- $50 is paid toward Vision services
Prescription Drug Card featuring:
- Generic: $10 for each 30 day supply
- Brand Name: $30 or $50 Copay once you meet a $2,000 drug
