- $30 CoPay for Physician Office Visits for
first 3 visits, per member, per year. Then 70% Co-Insurance (age 5 and under
not subject to deductible)
- Preventive Services (Lab testing,
Immunizations, Mammograms, etc 70% Co-Insurance, age 5 and under not subject to
Deductible)
- ER Visits: $500 CoPay (waived if admitted)
- Blue Cross POS Network of Doctors
- 70% Co-Insurance
- Out of Pocket Maximum: Deductible + $3000
- $5 million lifetime maximum benefit
- 9 month Initial rate guarantee
- No Maternity coverage
- Dental Insurance available (optional for additional
$27 a month indiv/$76 Family)

Prescription Drug Card featuring:
- Generic Drug Coverage: Greater of $15 Co Payment OR 40%
Coinsurance with NO drug deductible
- Brand Name Drug Coverage: $15/$30/$60 CoPay (or 40%,
whichever is greater) plus difference in allowable charge if brand is chosen
over an available generic, after you meet a separate $500 drug deductible. Out
of pocket maximum $4,000 per person per calendar year.
