- $30 CoPay for Physians Office Visit (first 3 visits)
- 100% Coverage for Preventive Care
- $500 CoPay for Emergency Room visit due to medical emergency (no
deductible or co-insurance)
- 70% Co-Insurance
- Out of Pocket Maximum In Network: Deductible + $3,000
- Unlimited lifetime maximum benefit
- Mail Order Rx available
- PPO network available for this plan with slightly different benefits at a
higher cost. (Call for details)
Prescription Drug Card featuring
Standard Drug Coverage:
- Drugs on Formulary (Generic and Brand Name/Specialty drugs): $15 CoPay or
40% Co-Insurance, whichever is greater.
- Drugs not on Formulary: NOT covered
Enhanced Drug Coverage:
- Tier 1: $15 CoPay; Tier 2: $30 CoPay; Tier 3: $60 CoPay; Tier 4: 40%
Co-Insurance with a $4000 out-of-pocket maximum per person per year. (If brand
name is chosen when generic is available, member must pay copay plus difference
